The Integrated Parkinson's disease scale assessment (UPDRS) is used to follow the longitudinal path of Parkinson's disease. The UPD assessment scale is the most commonly used scale in clinical studies of Parkinson's disease.

The UPDRS consists of these sections:

  • Part I: mentasi evaluation, behavior, and mood
  • Part II: self-evaluation of daily activities (ADL) including speech, swallowing, handwriting, dressing, hygiene, falling, salivating, spinning in bed, walking, and cutting food
  • Part III: evaluated motor evaluations measured by doctors
  • Part IV: therapeutic complications
  • Part V: Hoehn and Yahr's thinking about the severity of Parkinson's disease
  • Part VI: Schwab Scale and English ADL

It is evaluated by interview and clinical observation. Some parts require many values that are set for each extremity.

Doctors and researchers alike use UPDRS and motor parts especially to follow the development of Parkinson's disease. Scientific researchers use it to measure the benefits of therapy given in a more unified and accepted assessment system. Neurologists also use it in clinical practice to keep abreast of their patients' symptoms in a more objective way.

Following UPDRS scores from time to time provides insight into the progression of a patient's illness. For example, Michael J. Fox's symptoms start with a little vibration, so his motor score will be less than 10. For most patients, the "mentation, behavior and mood" score increases later on, but there is a section for those symptoms developed early.

Other scoring scales for Parkinson's disease are the Hoehn and Yahr and Schwab and UK scales of the daily life scale, although these two measures are currently included in the UPDRS in a modified format.

MDS-UPDRS

In 2007, the Disorder Society Movement (MDS) published a revised UPDRS, known as MDS-UPDRS. Revisions become desirable after the MDS-sponsored Task Force on the Assessment Scale for Parkinson's Disease highlights the limitations of the original UPDRS. Two major limitations include the lack of a consistent anchor between subscales and low emphasis on nonmotor features of the PD. The modified UPDRS retains a four-scale structure with reorganization of various subscales. Scales are now titled; (2) daily motor experience (13 items), (3) motor examination (18 items), and (4) motor complications (six items). Each subscale now has a rating of 0-4, where 0 = normal, 1 = little, 2 = light, 3 = medium, and 4 = weight.
